Common System Errors & Solutions
A quick reference guide for the most frequently encountered errors in the OLY system and how to fix them.
Permission Errors
"Insufficient Permission for [DocType]"
Cause: Your role doesn't have access to this document type. Fix:
- Check if you're logged into the correct account
- Contact your Department Head to request access
- IT Admin will update your Role Profile if approved
"Cannot Edit Submitted Document"
Cause: The document has been submitted (finalized) and is locked. Fix:
- Click Menu > Amend to create a corrected version
- Or ask a user with Submit permission to amend it
- Note: Not all doctypes support amendment
Data Errors
"Value Missing for Required Field"
Cause: A mandatory field was left blank. Fix: Fill in all fields marked with a red asterisk (*) before saving.
"Duplicate Entry"
Cause: A record with the same unique identifier already exists. Fix:
- Search for the existing record first
- Update it instead of creating a new one
- If it's a legitimate new entry, check the naming series
"TimestampMismatchError"
Cause: The document was modified by another user while you were editing. Fix:
- Refresh the page (Ctrl+R / Cmd+R)
- Re-apply your changes on the latest version
- Save again
Connection Errors
"NetworkError" or "Failed to Fetch"
Cause: Internet connection issue or server temporarily unavailable. Fix:
- Check your internet connection
- Try refreshing the page
- Wait 30 seconds and try again
- If persistent, contact IT Admin
"Session Expired"
Cause: You've been inactive for too long, or logged in from another device. Fix:
- Refresh the page — you'll be redirected to login
- Log in again with your credentials
- If using 2FA, have your authenticator app ready
Print/PDF Errors
"PDF Generation Failed"
Cause: The print format has an error, or the PDF engine is overloaded. Fix:
- Try a different print format (standard)
- Wait 1 minute and try again
- Check if special characters in the document might cause issues
- Report to IT if it persists
Email Errors
"Email Not Sent"
Cause: Email queue is full, or the email account has an issue. Fix:
- Check Email Queue (search bar → Email Queue)
- If your email shows "Error", check the error message
- Common causes: attachment too large, invalid recipient address
- Contact IT Admin for email server issues
General Troubleshooting Steps
- Refresh the page (Ctrl+R) — solves 50% of issues
- Clear browser cache (Ctrl+Shift+Delete)
- Try a different browser (Chrome recommended)
- Try incognito/private mode — rules out extension issues
- Check the URL — make sure you're on

When to Contact IT Admin
- Error persists after trying all steps above
- You see "Internal Server Error" (500 error)
- Data appears corrupted or missing
- You suspect a security breach
- System is extremely slow for extended periods
